These are the admittance forms we use at the clinic. Please print them off, and complete them BEFORE coming in for your visit. This will allow our doctors to spend the maximum amount of time on your treatment during that first visit!
GENERAL ADMITTANCE FORMS
- Clinic Admittance Form – All new patients must complete this form BEFORE their first appointment with the Doctors at Kinetic Health.
- Massage Therapy Admittance Form – All new massage patients must complete this form BEFORE their first massage appointment at Kinetic Health.
MVA FORMS
- Motor Vehicle Accident Form – All patients who have been involved in an motor vehicle accident must complete this form.
- Form AB-1 – Notice of Loss and Proof of Claim – To be sent to the insurance company after a MVA.
- Form AB-2 – Treatment Plan – To be sent to the insurance company after a MVA
